If you would like to register multiple children for a Sparrow’s Nest Play program, please fill out the form once for each child. After you’ve filled out the registration, scroll to the bottom of the page to download the participant waiver and the parent handbook.

After you register your child, please download the two forms below. The participant waiver can be filled out electronically and emailed to director@sparrowsnestplay.org, or can be printed off and then scanned and emailed.
After we receive your participant waiver, we’ll contact you about the registration fee. You can learn more about that by clicking the button below.
